How Prozac® Weekly™ (fluoxetine hydrochloride) Works
Prozac Weekly contains the same active ingredient as Prozac. It's simply a 90-mg dose that you only have to take once a week. The medication inside the capsule has a special coating that delays the release of the medication into your bloodstream. Because of the unique ingredients in Prozac Weekly, effective levels of the medication stay in the bloodstream for an entire week, safely treating depression.
Before taking Prozac Weekly, a person should be stabilized on 20-mg Prozac daily. Once 20-mg Prozac daily is working and symptoms have improved, a person may be ready to switch to Prozac Weekly. You and your doctor will choose a day to start Prozac Weekly. Your doctor may recommend that you start Prozac Weekly one week after you stop taking Prozac daily.
Things You Should Know If You're Taking Prozac® Weekly™ (fluoxetine hydrochloride)
The following is important safety information on Prozac Weekly and Prozac. Before starting therapy with Prozac Weekly, you should have been stabilized on 20-mg Prozac daily. In a clinical trial for Prozac Weekly, patients were treated for 13 weeks at 20 mg/day. Prozac Weekly, Prozac and Sarafem™ contain the same active ingredient -- fluoxetine hydrochloride.
If you develop a rash while taking Prozac Weekly or Prozac, call your doctor right away because this can be a sign of a serious medical condition.
Also, you should not take Prozac Weekly or Prozac at the same time as or within two weeks of stopping a type of antidepressant medication known as an MAO inhibitor. Don't take MAO inhibitors for at least five weeks after stopping Prozac Weekly or Prozac. Also, you should not take thioridazine at the same time as or within five weeks of stopping Prozac Weekly or Prozac.
Side effects are similar for Prozac Weekly and Prozac and may include nausea, difficulty sleeping, drowsiness, anxiety, nervousness, weakness, loss of appetite, tremors, dry mouth, sweating, decreased sex drive, impotence or yawning. These tend to go away within a few weeks of starting treatment and, in most cases, aren't serious enough to cause people to stop taking Prozac Weekly or Prozac.
Be sure to tell your doctor about any medications you may be taking (prescription, over the counter, herbals, etc). Your doctor can discuss safety matters with you in more detail.

Major depressive disorders, which include depression, affect an estimated 340 million people worldwide. If left untreated, the effects of depression can be devastating, robbing people of the energy or motivation to perform everyday activities and, in some cases, leading to suicide. Symptoms of the disorder include feelings of sadness or emptiness, lack of interest or pleasure in nearly all activities, and feelings of worthlessness or inappropriate guilt.

Clinical Results
In clinical trials, researchers examined 501 depressed subjects who responded well to 13 weeks of therapy with fluoxetine 20 mg. These subjects were then randomly assigned to treatment with a 90 mg enteric-coated formulation of fluoxetine taken once weekly, fluoxetine at 20 mg daily or a placebo for a 25-week, double-blind period.
Rates of relapse for subjects treated either with the enteric coated or 20 mg formulation of fluoxetine were significantly lower than for those treated with placebo. Additionally, subjects treated with either 90 mg or 20 mg fluoxetine rated significantly better on depression rating scales when compared with subjects assigned to placebo.
Side Effects
The most commonly observed adverse events associated with the use of fluoxetine in Untied States controlled trials for depression, obsessive-compulsive disorder and bulimia combined were the following:
Nausea
Headache
Insomnia
Anxiety
Nervousness
Somnolence (sleepiness)
Mechanism of Action
The antidepressant, antiobsessive-compulsive, and antibulemic actions of fluoxetine are presumed to be linked to its inhibition of CNS neuronal uptake of serotonin. Studies at clinically relevant doses in man have demonstrated that fluoxetine blocks the uptake of serotonin into human platelets. Studies in animals suggest that fluoxetine is a much more potent uptake inhibitor of serotonin than of norepinephrine.
Antagonism of muscarinic, histaminergic, and a1-adrenergic receptors has been hypothesized to be associated with various anticholinergic, sedative, and cardiovascular effects of classic tricyclic antidepressant (TCA) drugs. Fluoxetine binds to these and other membrane receptors from brain tissue much less potently in vitro than do the tricyclic drugs. (from Prozac Prescribing Information).
